Triggers for Identifying Palliative Care Patients and Identifying Resources

July 12 – October 24, 2021

Mission:

1. Establish a trigger that the facility will use to identify palliative care patients and establish a cohort.  
2. Develop a workplan.
3. Follow the cohort over the action period using the tracking tool.
4. Determine whether you can identify and use existing internal and external resources for these patients.
