Establishing a Plan to Manage Patient Symptoms and Improve Quality of Life

Mission:
- Understand and identify the different factors that contribute to symptoms in patients: physical, spiritual and psychosocial.
- How to assess and manage common symptoms associated with serious illness.
- Develop a workplan that specifically reflects an interdisciplinary care plan and/or symptom management orders.
- Access and complete Center to Advance Palliative Care (CAPC) modules for CMEs and further education and skill set of team members.
