Action Period 2: HOW

Establishing a Plan to Manage Patient Symptoms and Improve Quality of Life

October 25 – March 30, 2022

Mission:

  1. Understand and identify the different factors that contribute to symptoms in patients: physical, spiritual and psychosocial.
  2. How to assess and manage common symptoms associated with serious illness.
  3. Develop a workplan that specifically reflects an interdisciplinary care plan and/or symptom management orders.
  4. Access and complete Center to Advance Palliative Care (CAPC) modules for CMEs and further education and skill set of team members.
